The Importance of Case Managers

Case Managers are Decision MakersManaged care is today’s prevalent model for managing the process for delivering health care and managing its costs. Payers and insurers no longer simply write checks to pay for health care services ordered by a physician; they become involved in decisions about how much care a patient receives, what kind and by which providers.

The Triple Aim – “improving the individual experience of care; improving the health of populations; and reducing the per capita costs of care for populations” was first published by Berwick and his research team in 2008, and has become the standard by which care is being delivered and measured across the nation. So complexity is everywhere, and, with increases in aging and chronically ill people, there is a critical need for a new approach to care.

Americans are living longer and the workforce is aging – meaning that there is an urgent need for highly skilled care managers and care coordinators. Care Excellence is a practical and interactive training solution, delivering a multi-pronged approach to train and retain care managers, and enhance their skills.

Care Excellence helps care managers meet the needs of large, diverse populations with special and complex needs. Funded by the California Health Care Foundation and The SCAN Foundation, the Care Excellence curriculum is designed to train care managers from various practice settings.

You may access the wide range of curriculum by purchasing the entire series, an individual course, or a specific module.

Care Excellence Foundational courses from May until December of 2016. The Leadership courses will begin in June and continue to roll out through the end of the year.
