Case Management Training to Elevate the Skills of the Care Manager

    A health plan care managers typically have worked in the past with people with complex medical problems. The way that care managers need to help people with heart failure or emphysema is very different than the way a care manager needs to work with someone who has mental illness, who substance use disorder, or unstable housing, or off in a combination of all those three things. Health plans trying to find care managers will find people who are experts in heart failure and emphysema, but absolutely do not know what to do with someone who is using methamphetamine everyday, or is smoking five packs a day, or is unable to find stable housing.

    What care excellence does is it helps care managers whether they are new or experienced get the set of skills to understand the complex social environment where people are living, the social services available in different communities. Most importantly, it is an attitude shift, instead of doing things for patients, or telling people what to do, or directing them. The successful care manager has very different skills focused on how to gain trust, how to earn somebody’s trust, how to help them maybe do something for them the first time, then do it together the second time, then finally have that patient be able to do things on their own and lean back on you for help. This coaching model is very different than any healthcare expert model. A coaching model has the opportunity to give skills for people that will last a lifetime, as opposed to just getting them help at that moment and then having things fall apart when you graduate from the program.
